Position Summary:

The Community Engagement Representative will directly assist the Director of Community Engagement in expanding Community Care Plan’s presence and brand awareness. This position will engage with community leaders, community, and faith-based organizations to collaboratively create meaningful and intentional engagement for the benefit of both the community and Community Care Plan (CCP). Position duties will be primarily performed in a specified territory.


Qualifications:

  • Bachelor’s Degree in Public Health, Healthcare Administration, or other related Health Sciences with at least one year of relevant experience (preferred).
  • Must reside in the community in which the position is located. (Newar the Orlando Area)
  • Knowledge of the local community(ies), and experience working with local non-profit and/or faith-based organizations.
  • Microsoft Office and internet software proficiency.
  • Must possess a valid unrestricted Florida drivers’ license.
  • Bi-lingual (preferred).

  • Experience delivering presentations to varied audiences.
  • Ability to work independently.
  • Ability to meet deadlines.

  • Skilled in maintaining good rapport and collaborative working relationships with team members.

Essential Duties and Responsibilities:

  1. Actively searches for opportunities to maximize partnerships with community and faith-based organizations (FBO) that serve CCP’s targeted populations.
  2. Develops community knowledge and builds partnerships and coalitions, including Community & Faith Based Organizations (CBO’s), that align with CCP’s mission.
  3. Conducts meetings with key local stakeholders to recognize community needs as well as communicate the mission and goals of Community Care Plan.
  4. Identifies key action items that CCP will execute to ensure that new relationships remain meaningful and mutually beneficial.
  5. Educates Community Based Organizations (CBOs) on CCP’s Expanded Benefits and Healthy Behavior programs to ensure eligible CCP members are linked to appropriate programs as identified.
  6. Travel to the CCP (Sunrise location) office on a quarterly basis or as needed (up to twice per year, minimum) to engage with CCP’s Community Health team for integration of workflows, process alignment and continuous improvement initiatives.
  7. Represents Community Care Plan at local partner meetings, conferences, and events, including but not limited to health fairs, to create awareness of Community Care Plan and gain insight into the unique needs of each community.
  8. Assists with logistics for community listening sessions, in coordination with CCP’s Marketing Team, including identifying contacts, location and other needs as identified.
  9. Assists with execution of identified action items as needed to ensure success.
  10. Maintains active communication with the Director of Community Engagement on results of outreach efforts and requests assistance when needed.
  11. Facilitates handoff to various CCP departments including Community Health and Concierge Care Coordination to ensure community partnerships are recognized and utilized.
  12. Provides data and outcomes for reporting and presentation needs.
  13. Promote awareness of CCP’s Community Resource Center, particularly the virtual site to the community.
  14. Conducts telephonic outreach to support strategic community engagement initiatives focused on addressing Health-Related Social Needs (HRSN), including connecting members to community resources such as food, housing, employment, and other priority needs. May provide support with Health Risk Assessment (HRA) completion during outreach efforts, as needed, supporting improved member experience and retention across the member journey.
  15. Documents outreach activities and collaborates with internal departments to support member linkage to community resources and track outcomes related to HRSN engagement efforts.
  16. Identifies potential resources and events to promote on CCP’s Community Resource Center website.
  17. Monitors and evaluates outreach efforts for process improvement.
  18. Performs other related duties as assigned.
