Position Summary

Contact youth and families by telephone to introduce the OhioRISE Care Management Entity (CME), explain OhioRISE and obtain verbal consent to participate in care coordination when appropriate. Document all outreach attempts and communication in accordance with OhioRISE, Medicaid, and organizational documentation standards.

Key Responsibilities

Support Care Coordinators and supervisors with the management of initial activities.

Review documentation for completeness and accuracy.

Maintain confidentiality of member information.

Complete special projects assigned by leadership.

Adhere to OhioRISE organizational policies.

A successful OhioRISE CME Intern demonstrates professionalism, compassion, strong communication, dependability, and a willingness to learn while maintaining confidentiality and adhering to all OhioRISE CME organizational policies.
