CareOne Home Healthย is seeking a Clinical Support Associate for our growing home health business in Salida, CA. The Clinical Support Associate is responsible for a wide variety of clerical duties, including data entry, provider scheduling, maintaining patient records, daily mail duties and special projects as needed. Involved in triage coverage and scheduling, data management, tracking and performance reporting, meeting scheduling and coordination, department phone coverage, the coordination and processing specific work queues. Serves as an internal department office resource for Provider team to address questions and resolve issues in collaboration with managers and directors.

Requirements:
- 1 year related experience, home health experience preferred
- Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
- Experience with Kinnser (WellSky) EMR preferred
- Ability to manage multiple tasks/projects and deadlines simultaneously
- Excellent interpersonal, organizational and communication skills
Responsibilities:
- Responsible for providing administrative support, as needed all Home Health Field Staff; Nurse Case Manager, LVN, CHHA, Physical Therapist/Assistant, Occupational Therapist, Social Services, Speech Therapy, Care Coordinator, Clinical Coordinator.
- Works to ensure on-call service is covered appropriately
- Coordinates assists with oversight of field staffing schedules
- May schedule patient visits as directed
- Daily log into the phone queue to answer phone calls
- Faxes pertinent information as directed
- Work closely with the Triage staff and Nursing staff as necessary
- Manage Medical Records
- Maintains daily inbox with scheduling requests that impact provider team workflow
- Manage master schedule for team working within their assigned territories and add to weekly visit schedule for new and existing patients
- Follow HIPAA guidelines when accessing and sharing patient information
