We are a growing home care agency seeking a Clinical / Intake Coordinator to support daily operations under the Administrator. This role is responsible for managing new patient intakes, coordinating care services, and ensuring compliance with clinical and regulatory standards.

Key Responsibilities:

  • Handle all incoming patient referrals and intake processes
  • Verify insurance and coordinate service eligibility
