Job description

Hospice Services of Massachusetts is looking for a Clinical Intake Coordinator to join our growing hospice agency!

This is an in-person position based out of our Lakeville office. Full-time role paying $37-$40 an hour, with weekly pay. Must be an LPN or RN.

As the Clinical Intake Coordinator, you oversee the patient intake process for our agency, including maintaining positive relationships with our referral sources and responding to requests and concerns.

Requirements:

  • Must possess an active and unencumbered LPN or RN license in Massachusetts.

  • Prior experience in hospice or home health preferred.

Duties:

  • Receives and accepts all daily patient referrals through phone, fax, and electronic solutions.

  • Ensures compliance with all state and regulatory requirements are met with information received.

  • Use work methods and procedures that ensure patients are admitted in accordance with policy.

  • Establishes and maintains positive working relationships with current and potential referral sources.

  • Builds and monitors community and customer perceptions of the agency as a high-quality provider of services.

  • Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources.

  • Documents physician orders for intake/referrals accepted by all intake coordinators as well as own.

  • Assures timely completion of all documentation to assure that patients are admitted timely.

  • Follows up with referral source to check status of pending referrals.
