Reports To: Clinical Director / Administrator
Position Summary
Our Home Health and Hospice Agency is seeking a compassionate and dedicated Chaplain to provide spiritual and emotional support to patients, families, and caregivers. The Chaplain serves as a member of the interdisciplinary team and assists patients in addressing spiritual, religious, emotional, and existential concerns during illness, end-of-life care, and bereavement.
The Chaplain will respect diverse religious traditions, beliefs, and cultural backgrounds while providing spiritual care that enhances the quality of life for patients and their loved ones.
Essential Duties and Responsibilities
Spiritual Care Services
  • Conduct spiritual assessments of patients and families.
  • Provide spiritual counseling and emotional support based on patient preferences and beliefs.
  • Assist patients and families in addressing spiritual, religious, and end-of-life concerns.
  • Facilitate prayer, scripture readings, sacraments, memorial services, and other religious practices when requested.
  • Offer support during times of crisis, grief, loss, and transition.
  • Provide comfort and encouragement to patients facing serious illness or end-of-life issues.
Interdisciplinary Team Participation
  • Participate as a member of the Home Health and Hospice interdisciplinary team.
  • Collaborate with physicians, nurses, social workers, therapists, and other team members to address patient needs.
  • Contribute spiritual care information to patient care plans.
  • Attend interdisciplinary team meetings and case conferences.
Family and Bereavement Support
  • Provide counseling and spiritual guidance to family members and caregivers.
  • Assist families with grief and bereavement support.
  • Participate in memorial services and bereavement programs as appropriate.
  • Support families before, during, and after the patient's passing.
Community Outreach
  • Establish relationships with local clergy, churches, faith-based organizations, and community resources.
  • Coordinate spiritual support services with community religious leaders when requested by patients or families.
  • Serve as a resource regarding spiritual care and end-of-life issues.
Documentation and Compliance
  • Maintain timely and accurate documentation of spiritual care visits.
  • Complete required assessments, care plans, and progress notes.
  • Comply with Medicare, Medicaid, HIPAA, state regulations, and agency policies.
  • Maintain patient confidentiality at all times.
Education Requirements
Required
  • Bachelor's Degree in Theology, Divinity, Pastoral Ministry, Religious Studies, Counseling, or a related field.
Preferred
  • Master of Divinity (M.Div.), Master of Theology, Master of Pastoral Ministry, or equivalent graduate theological degree.
Experience Requirements
Required
  • Minimum one (1) year of pastoral, ministry, chaplaincy, counseling, or spiritual care experience.
Preferred
  • Two (2) years or more of Hospice, Home Health, Hospital, Long-Term Care, Nursing Home, or Healthcare Chaplain experience.
  • Experience working with terminally ill patients and their families.
  • Knowledge of grief counseling and bereavement support.
  • Experience working with individuals from diverse faith traditions and cultural backgrounds.
Qualifications
  • Strong spiritual counseling and interpersonal skills.
  • Ability to provide compassionate, non-judgmental care.
  • Excellent verbal and written communication skills.
  • Ability to work independently and as part of an interdisciplinary team.

Licenses and Certifications
Preferred
  • Board Certified Chaplain (BCC)
  • Certified Hospice and Palliative Care Chaplain (if applicable)
  • Ordination, ecclesiastical endorsement, or ministerial credentials from a recognized faith organization
Required
  • Valid Texas Driver's License
  • Reliable transportation and current automobile insurance
Physical Requirements
  • Ability to travel throughout the service area to patient homes, facilities, and hospitals.
  • Ability to sit, stand, walk, bend, and lift up to 25 pounds occasionally.
  • Ability to provide services in various patient care settings.
