Summary

The Field-Based Case Manager will be responsible for providing direct program implementation activities as an integral part of the agencyโ€™s Behavioral Health and Support Services unit under the direct supervision of the Licensed Director of Behavioral Health. Responsibilities include outreach and recruitment; conducting participant screenings and assessments; facilitating trauma-informed, culturally affirming individual and group level counseling; providing targeted case management; and tracking participant progress. This is a field-based community-mental health service program requiring off-site travel to various locations. The Case Manager will be exposed to the five core program pillars of the division including (1) sexual health, (2) behavioral health [inclusive of mental health and addiction), (3) housing, (4) forensic treatment, and/or (5) human trafficking. Must be able to work with diverse ethnic and cultural communities, including but not limited to, the following vulnerable populations: communities of color, queer, gender non-conforming LGBTIA+, monolingual or limited English speaking-undocumented immigrants and refugees, massage parlor workers, people living with or at risk for HIV/AIDS and STIs, people experiencing substance use disorders, homeless, the chronically unhoused, folks recently released from incarceration, victims of human trafficking, of varying ages and socioeconomic status.

Essential Functions

1.ย ย ย ย ย ย  Conduct client intake screenings into behavioral health and psychosocial support program services, and assessment plans.

2.ย ย ย ย ย ย  Develop and implement outreach, recruitment, and linkage referrals to community-wide stakeholders.ย 

3.ย ย ย ย ย ย  Provide targeted case management services, such as referrals to food assistance, childcare, transportation, housing; care coordination; employment development; skills building.

4.ย ย ย ย ย ย  Sees FSP clients in the community, including housing sites and OSP clients in the office, including unhoused individuals.

5.ย ย ย ย ย ย  Goes into the field to take FSP clients to appointments, etc., travels with them.

6.ย ย ย ย ย ย  Provide necessary program documentation, including the completion of reports, administration of evaluation tools, and other program related assignments.

7.ย ย ย ย ย ย  Facilitate group level counseling sessions.

8.ย ย ย ย ย ย  Attend and actively participate in unit, case conference, internal staff in-service, community, and other relevant agency events.

9.ย ย ย ย ย ย  Ability to work overtime and perform duties during weekends, holidays, and evenings when required.

10.ย ย  Maintain and uphold Agency mission statement, values, policies, procedures, and principles.

11.ย ย  Other activities as required to conduct program and agency objectives.

12.ย ย  Regular attendance required.

Minimum Qualifications - Knowledge, Skills and Abilities Required

  1. Valid CA Driverโ€™s License and Proof of Car Insurance

2.ย ย ย ย ย ย  High school diploma or GED equivalent and above, or at least 2 years of experience in the HIV/AIDS or behavioral health field.

3.ย ย ย ย ย ย  Fluent in Spanish.

4.ย ย ย ย ย ย  Knowledge of HIV/AIDS, and related issues.

  1. Knowledge of and ability to work with racial/ethnic minority, impoverished and underserved communities and groups (e.g., multi-gender, generation, LGBTQ, substance using).

6.ย ย ย ย ย ย  Proficient writing and oral communication skills.

  1. Demonstrated experience and knowledge of PC hardware and software (i.e., Windows, word processing, spreadsheets).
  2. Ability to coordinate multiple tasks simultaneously in a high-pressure environment.
  3. Maintain and uphold agency mission statement, values, policies, procedures, and principles.
  4. Valid CA driver's license, auto insurance, and reliable transportation.
  5. Verification of employment eligibility and background check.
  6. Updated tuberculosis test (annual requirement) โ€“ read, reviewed, and dated by a clinician 6 months prior to employment.

  1. Experience working in a non-profit community-based organization.
