Community Care is hiring a Case Manager/Care Manager Day Shift for our Family Care Program in Milwaukee, WI. Offering an $8,000 sign-on bonus! This is a Monday through Friday position, 8:00 a.m. to 4:30 p.m.
This rewarding opportunity is perfect for someone who enjoys team collaboration, community nursing case management, being part of a mission driven organization and who shares our mission to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. As we continue to grow our team, we are easing caseloads to bring even more work life balance to our valued employees.
Case Manager/Care Manager Job Responsibilities:
• Manage caseload of Family Care Program members
• Complete comprehensive clinical assessments
• Working collaboratively to develop, member centered care plans, provide on-going care coordination, member education, quality assurance and monitoring of services.
• Travel, Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
Case Manager/Care Manager Job Requirements:
• Education: Bachelor's Degree in Human Services or closely related field required.
• Licensure: Current long-term care functional screener certification preferred (eligibility for this certification is required)
• Experience: Minimum of 3 years recent experience with older adults or persons with physical or intellectual disabilities is required. 3 years of work in community nursing preferred.
