The Care Coordinator creates the unique care plan, manages the care, and provides constant feedback to clients, clients representatives and caregivers. They work closely with our office team and have excellent communication and problem-solving skills.

Requirements

  • Upbeat Personality
  • Master in Social Work, Nursing or Relevant Field, preferred
  • Industry Experience, preferred
  • Strong Computer Skills
  • Strong Organizational & Communication Skills
  • Pass Live Scan Fingerprint Clearance (LIC 9163)
  • Valid driver's license and proof of insurance required
  • Cell phone with Internet Access
  • Proof of Negative TB results
  • Willing to Work One Weekend a Month as Back-Up On Call
