The Care Manager is responsible for assuring the quality of service provided to team members, coordinating the day-to-day operations of the team, and recommending and implementing changes that would improve the delivery of services. The Care Manager works with a panel of high-risk patients, one-on-one, to develop and manage their individual care plans. S/he is accountable for coordinating care in partnership with the care team and across the care continuum. This includes consideration of social risk and other issues.
TASKS & RESPONSIBILITIES
โข Management of beneficiary needs during transitions of care.
โข Management of rare diseases and high-cost procedures.
โข Consult with identified professionals, family members, and others, include their input in the Person-Centered Planning process, inform all involved stakeholders, and monitor the status of the consumer relationships in relation to treatment goals.

โข Development and deployment of prevention and population health programs
โข Coordination of services
โข Documentation and documentation management/monitoring
โข Fully participates in agency outcome evaluation system. Ensures the collection of required data
โข Care delivery monitoring
โข Coordinates and participates in clinical case conferences, when needed.
โข Adheres to the Professional Code of Ethics and complies with the State of North Carolina Division of Mental Health, Developmental Disabilities, and Substance Abuse.
โข Coach staff in working with consumers and provide training to other employees by demonstration, repetition, modeling, large group training, etc.
โข Responsible for the development, implementation, monitoring, and revision of the Person-Centered Plan (PCP) as it relates to the needs of the individual
โข Assist with the coordination of new admissions and transfers (e.g.) screening applicants and facilitating placement, etc.
โข Assist in audits if needed
โข Other duties as assigned

EDUCATION & REQUIREMENTS
โข A bachelorโs degree is required in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area, or licensure as a registered nurse (RN).
โข At least 18 years of age
โข Graduated from a four-year college or university
โข Able to read, write, understand, and follow directions
โข No substantiated findings of abuse or neglect listed on the North Carolina Health Care Personnel Registry
โขKnowledge of Client Rights
โขAbility to handle crisis/emergency situations, and utilize conflict resolution skills
โขKnowledge of basic computer and applications including Microsoft Word
EXPERIENCE
โข Two years of experience working directly with individuals with behavioral health conditions or with an I/DD or a TBI need.
