This position is fundamental to provide primary care services across a broad spectrum of populations including newborns, infants, children, adolescents, adults, pregnant and postpartum women, and older adults. The family practice care team collaborates with Professional Colleagues to ensure the highest level of patient care involving a focus on the assessment, diagnosis, and management of both health and illness, and the prevention of disease and patient wellness ness programs. The care team, in conjunction with the Medical Director focuses on population health in the region, supports the safety net aspect of serving the vulnerable, and in conjunction with associated State and Federal Programs offered in the health center.
