Description
We are looking for a Director of Case Management at Howard Memorial Hospital. We are a critical access hospital located in Nashville, Arkansas. We provide comprehensive patient-focused care in a variety of settings- emergency, surgical, inpatient and outpatient-delivered by skilled medical professionals. Our not-for-profit status ensures that all financial resources remaining after expenses are re-invested in the hospital to improve technology, the facility and patient services. 
This position is an integral role in providing case management/utilization review, and discharge planning. It is a full-time, 72 hours per pay period position.
Responsibilities
  • Provides case management/utilization review and discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment.
  • Provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs.
  • Coordinate the integration of social services/case management functions into the patient care, discharge, and home planning processes with other hospital departments, external service organizations, agencies, and healthcare facilities.
  • Communicate with physicians at regular intervals throughout hospitalization and develops an effective care plan to maintain. appropriate cost, case and desired patient outcomes.
  • Facilitates interdisciplinary patient care conferences to review treatment goals, optimize resource utilization, and identified post-hospital needs.
  • Assess patient progress throughout hospital stay, while serving as patient and family advocate.
  • Collaborate with clinical staff in the development and execution of the plan of care, and achievement of goals.
  • Supervises the duties and schedule of the Utilization Review Nurse.
Qualifications
  • BSN preferred, ASN required.
  • 3+ years experience in clinical nursing experience.
  • Effective written and verbal communication and critical thinking skills.
  • Readily exhibits leadership skills.
  • Obtain Case Manager certification within 5 years of entry into the position.
