Job description

The Assistant Director of Nursing is responsible for ensuring quality patient care via development, implementation, and evaluation, of individual patient care plans in the home setting. The Assistant Director of Nursing serves as the liaison between the Agency, DON/Clinical Manager, Operations Manager, field staff and patient families. They are responsible for day-to-day management, coordination, and supervision of the related clinical activities utilizing professional standards of care and Agency Policy & Procedures. The Assistant Director of Nursing reports directly to the Operations Manager and DON/Clinical Manager. He or she supports and creates positive environments through a personal commitment to the Companyโ€™s vision and values while demonstrating and maintaining strong leadership skills.

Essential Functions:
  • Provide Oversight of care implementation within nursing skilled team and the Scheduling Department with attention to quality patient care and patient family satisfaction.
  • Coordinates care authorization with Authorization Specialists and Case Managers, including new admissions and re-certifications.
  • Ensures proper implementation, evaluation, and oversight of all assigned clients and their Plan Of Care with field staff.
  • Works with management team to ensure that state regulations are upheld within designated field office.
  • Knowledgeable of relevant chapters in the EEOICP Manual and provide education to field staff
  • Performs quarterly Quality Assurance Visits and assists Authorizations and CMโ€™s with an ongoing complete DOL Benefits Analysis for the assigned client(s)
  • Build rapport with patient families and skilled nursing staff in implementing and managing care being rendered.
  • Works alongside Staffing Coordinator to schedule work load for maximum efficiency as needed and remains detail oriented to deadlines and cost efficiency.
  • Plans and conducts orientations and continuing education for staff engaged in patient care as needed
  • Maintains communication including a weekly update with Operations and DON/Clinical Manager, Case Managers, RN Authorization Specialists, non-licensed staff, support staff, and ancillary services.
  • Coordinate with Quality Assurance staff in implementing staff development as required by maintaining documentation compliance.
  • Demonstrates positive attitude and exhibits appropriate behaviors expected to maintain and promote a cohesive work environment.
  • Provide clinical guidance and coaching for all clinical field staff, clients and families.
  • Orchestrates, with the oversight of Operations and DON/Clinical Manager, Case Management meetings occurring at the very least on a monthly basis and meetings with other lower level field staff PRN
  • Builds and maintains professional working relationships with members of management and care teams.
  • Participates in required Company in-service/education programs as scheduled.
  • Monitor QuickBase for accuracy, updated and timely documentation/information by case managers; ensure 60-day assessments are completed by deadline and reviewed for proper documentation for all assigned clients.
  • Participates in designated company committees per direction/instruction.
  • Participates with Operations and DON/Clinical Manager as needed per their direction in case Manager job performance reviews, coaching and counseling.
  • Participates in home visits in service area per Policies and Procedures and as needed to resolve field issues as they occur.
  • Abides by and promotes all Agency Policies & Procedures.
  • Conduct initial assessments and assist with on-boarding process of new clients under the direction of Operations, DON/Clinical Manager, and On-boarding Coordinator as needed.
  • Demonstrates effective delegation of tasks, working with and through others as needed as directed by Operations and Clinical Manager.
  • Monitors in-home staff education requirements (including HHA tests) to ensure regulatory compliance.
  • Supports and takes initiative on necessary state and federal program regulations by process development, implementation, and evaluation of compliance for said regulations.
  • Maintains a continuing knowledge of competencies related to the Nursing profession by participating in formal education programs, conference, workshops, and professional organizations.
  • Participates in management and leadership team and attends to corresponding responsibilities per company development and growth.
  • Treats current and perspective company clients and families with absolute customer service and compassion with close communication, friendly service, and the upmost professionalism.
  • Available and present during company office hours (unless pre-arranged with DON/Clinical Manager or Operations) to enable accessibility of service for potential and current patients and families as well as staff and colleagues.
  • Provides/ensures effective coverage with communication to all parties involved related to any changes in prior approved schedules.
  • Participates in performance review and goal setting for growth and company development in assigned territory.
  • Acquires and provides to the team the detailed information regarding an assigned client to establish the foundation for treatment Plan of Care.
  • Fills any role as needed in the event of an emergent situation for any given client in the event the shift is unable to be filled.
  • Maintains open communication with clients and staff; providing necessary information as it relates to overall client care; clientโ€™s DOL benefits, Authorization team, Chart Review, billing departments.
  • Participates in public relations and community activities that promote RAHโ€™s role as an effective provider of inโ€‘home care in the DOL OWCPโ€™s programs
  • Ensure the structure and chain of command is observed by all nursing staff (per Organizational Chart)

Education, Training, and Skill Recommendations/Requirements:
  • Current RN licensure in state(s) deemed necessary by the Company
  • Minimum 3 years progressively responsible clinical experience
  • Excellent writing, organizational, and time management skills
  • Prior management or leadership experience necessary
  • Ability to work independently, multiโ€‘task, prioritize, and function in a team environment
  • Must possess a professional, courteous, compassionate and customerโ€‘oriented demeanor
  • Must be discrete, trustworthy, and possess a high level of integrity
  • Excellent computer and internet skills
  • Must be able to do bending and lifting required for effective patient related care
  • Must be able to safely lift 40-50 lbs.
