Job description

Job Summary:
  • Evaluate and admit patients to hospice care, develop a plan of care, and provide care to the patient according to plan.
  • Provide end-of-life care and coordination at time of death and bereavement support following death.
  • Act as case manager making necessary adjustments in the patient's plan of care related to symptom changes and changes in care needs.
  • Act as liaison for patient/caregiver with MD, community health care partners and other hospice team members.
  • Complete all documentation related to patient contact, assessments, and IDT meetings according to policy.
  • Provide on-call coverage for both home care and hospice as assigned.
  • Make necessary home care visits as assigned.
  • Demonstrate and encourage team behavior and exceptional patient/guest experiences.
  • Uphold and promote patient safety and quality.

Education:
Associate's degree in nursing and currently licensed in the state of Alabama required.
Experience:
Minimal 2 years' experience in a hospital setting working in medsurg, critical care or ER. Previous home care or hospice experience preferred.
Additional Skills/Abilities:
Must exhibit strong critical thinking and organizational skills. Must be able to work independently and make independent decisions. Must be flexible in scheduling. Must be able to organize and prioritize a schedule to meet patient needs.
