Registered Nurse (RN) - Case Manager- Days

Schedule: PRN, Day Shift

Responsibilities
  • Consult regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfers, and discharge.
  • Monitor patient and family satisfaction; respond to questions and complaints from patients, families, and payors.
  • Lead and coordinate discharge planning, including patient transfers to other facilities and referrals to community resources; provide discharge education.
  • Perform chart reviews to identify and address issues related to service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
  • Communicate effectively with providers, payors, and ancillary services to remove barriers and ensure timely care progression.
  • Accurately document case management activities, plans, and outcomes.
  • Participate in performance improvement and required continuing education.
  • Support department operations by working alternate shifts/units as needed; may be assigned on-call.
  • Perform other duties as assigned.

Schedule: PRN – Day Shift (8:00 am – 4:00 pm)

Qualifications
  • Current Alabama or compact state Registered Nurse license.
  • Associates degree from an accredited nursing school.
  • American Heart Association (AHA) Basic Life Support certification required within 30 days of hire.
  • AHA ACLS and PALS certification required within six months of hire.
  • One year of experience preferred.
  • Critical thinking skills, decisive judgment and ability to work with minimal supervision; capable of functioning in a stressful environment and taking appropriate action.
