Job description

A Director of Case Management opportunity is available in Bradenton, FL. This leadership role oversees all operational aspects of hospital case management, bringing executive-level direction to care coordination, discharge planning, and clinical resource utilization across the department.
The role. Reporting to senior clinical and financial leadership, this director sets the strategic and operational direction for the case management department, including staffing models, skill mix, FTE allocation, and staff competencies. The director ensures rigorous adherence to care coordination and discharge planning processes — with a focus on timeliness, quality, and documentation integrity — while analyzing performance data to drive and sustain improvement initiatives. A key part of this role is cultivating strong cross-functional relationships with the CMO, CNO, ancillary service leaders, and compliance and legal partners to align case management practice with broader organizational goals. The ideal candidate brings executive presence, proven change management capability, and a track record of delivering measurable outcomes through interdisciplinary collaboration.
What we're looking for.
• Bachelor's degree, required
• Master's degree in Nursing, Health Administration, or Business Administration, preferred
• Either Registered Nurse (RN) licensure, Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW), required
• 3+ years' of recent experience in overall acute care Hospital Case Management required
• 2+ years' experience in case management leadership
