POSITION TITLE: ย DIRECTOR OF CASE MANAGEMENTย 

REPORTS TO: ย Chief Program Officer

SUPERVISES: ย Clinical Supervisorsย 


POSITION SUMMARY: ย 

This position leads Caracole's Case Management program and oversees and manages all aspects of planning and service delivery of Case Management services. ย The goal is to provide best practice case management services for individuals living with HIV.ย 


RESPONSIBILITIES:ย 

  • Assure that the mission of Caracole is evident in Caracole's Case Management programs. Lead the case management team in planning for, implementing and evaluating best practice, effective case management services.ย 
  • Participate with agency leadership in strategic planning across all programs and services. ย Identify and document gaps in services and participate in developing strategies to resolve them. ย ย 
  • Hire and supervise Clinical Supervisors. Monitor and oversee professional practice of Clinical Supervisors. Assist Clinical Supervisors in providing oversight for their Case Management staff.ย 
  • Serve as liaison for funders and partners for case management; participate in outreach efforts to promote Caracole's services and to develop new partnerships. ย 
  • Review and interpret budgets, grant documents, contracts and regulations governing the performance of funded services.ย 
  • Prepare budgets, workplans, grant applications and grant outcome and compliance reports for Caracole's case management programs.ย 
  • Develop and revise documentation systems as needed to improve client services and promote efficiency. ย ย 
  • Coordinate data collection; ensure completion and timely submission of required reports. Utilize data to assess efficacy of service delivery and staff performance.ย 
  • Assist with the coordination of staff education and training.ย 
  • Assist with the coordination of Continuous Quality Improvement activities. ย 
  • Identify and pursue educational opportunities to promote professional growth.ย 
  • Represent Caracole at local and state conferences, committees, etc.ย 
  • Complete other duties as assigned by the Chief Program Officer.

Requirements

QUALIFICATIONS: ย ย ย 

ย Bachelor's degree is required. LISW-S Preferred. Must have an extensive understanding of case management practice. Three to five years of managerial and/or supervisory experience in social services and/or public health, including planning, quality assurance, regulatory compliance, and grants management. Proven ability to work harmoniously with ethnically, culturally, and racially diverse groups of individuals, establish strong community linkages, lead, teach and model good social work practice. Demonstrated experience working with individuals living with chronic disease. ย ย ย 


ย REQUIRED SKILLS:ย 

  • Change management, leadership, management and supervisory skills with high emotional intelligence.ย 
  • Demonstrated experience with individuals living with HIV/AIDS, Substance Use Disorder or other chronic disease.ย 
  • Ability to maintain confidentiality in verbal, print, and electronic communications.ย 
  • Advanced computer skills, including skills in Microsoft Word and Excel and Adobe software. ย Ability to use excel and other tools for data analysis
  • Read, write, speak and understand English with excellent verbal and written communication skills.ย 
  • Sensitivity to persons living with chronic disease (HIV/AIDS and/or addiction) and co-existing social, mental and physical conditions.ย 

PHYSICAL REQUIREMENTS: ย 

This hybrid, full time position includes time spent working in office, remotely at home, and in the community. Work is generally 9-5 M-F. The employee must be able to remain in a stationary position for multiple hours. Must operate a computer and other office productivity machinery, such as a phone and copy machine. Must have the ability to accurately communicate information and ideas so others will understand. Must be able to move throughout the building and other sites in the community. Employee must have reliable transportation and ability to drive to other sites and meetings as needed. The employee must be able to lift and/or move up to 10 pounds. ย ย ย 

SALARY:

This position is full time, salary (exempt). The pay range is $86,500 - $93,000 annually based on licensure and experience.
