The Opportunity

We are currently seeking a dedicated Admissions Nurse - RN to join our team. The Admissions Nurse is responsible for coordinating all clinical aspects of the admission process, ensuring a smooth transition for new residents while maintaining compliance with all federal, state, and facility regulations. This position serves as a clinical liaison between hospitals, referral sources, residents, families, and the interdisciplinary team.

What You Will Do
  • Coordinate and facilitate all clinical aspects of the admission process for new residents.

  • Review hospital records, physician orders, medications, and diagnoses prior to admission.

  • Conduct comprehensive admission assessments and develop individualized care plans.

  • Collaborate with the Admissions Director, Business Office, Therapy, MDS, Social Services, and Nursing departments to ensure seamless admissions.

  • Communicate with hospitals, physicians, case managers, referral sources, residents, and families throughout the admission process.

  • Ensure all admission documentation is complete, accurate, and entered into the electronic medical record.

  • Verify insurance authorizations and collaborate with insurance providers as needed.

  • Coordinate room readiness and communicate admission information to the nursing team.

  • Educate residents and families regarding the admission process, services, and expectations.

  • Monitor residents' progress following admission and communicate any changes to the interdisciplinary team.

  • Ensure compliance with CMS regulations, state regulations, Harborview policies, and facility admission requirements.

  • Participate in census development and clinical marketing efforts when appropriate.

  • Maintain accurate and up-to-date documentation of resident interactions and care plans.

  • Perform duties as assigned.

What You Must Have
  • Active Registered Nurse (RN) license in good standing in the state of practice.

  • Skilled Nursing Facility or Long-Term Care experience preferred.

  • Previous admissions, MDS, case management, or care coordination experience preferred but not required.

  • Strong clinical assessment and critical thinking skills.

  • Excellent communication and organizational skills.

  • Ability to work collaboratively with physicians, hospitals, referral partners, residents, families, and the interdisciplinary team.

  • Knowledge of Medicare, Medicaid, Managed Care, and skilled nursing admission requirements is preferred.
