Job description

The Transition Youth Program Manager is responsible for the development, implementation, coordination, and oversight of services for transition-age youth (typically ages 16–25) receiving care through the Federally Qualified Health Center (FQHC). This position provides leadership for integrated, trauma-informed, and culturally responsive programs that support youth with behavioral health needs.

The Program Manager collaborates with interdisciplinary care teams, community organizations, schools, social service agencies, and external partners to improve access to comprehensive services, care coordination, health outcomes, and patient engagement. The position also oversees program performance, quality improvement initiatives, deliverables, and compliance with HRSA, state, and organizational requirements.

 Responsibilities:

  • Develop, implement, and manage transitional youth programs and initiatives.
  • Supervise program staff.
  • Establish policies, workflows, and best practices for transition planning.
  • Coordinate interdisciplinary care for youth with medical, behavioral health, and social needs.
  • Build collaborative relationships with community partners, educational institutions, child welfare agencies, and behavioral health providers.
  • Ensure compliance with HRSA, HIPAA, FQHC regulations, and accreditation standards.
  • Participate in quality improvement initiatives using data-driven methodologies.
  • Promote culturally competent, trauma-informed, youth-centered care.
  • Bachelor's degree required; master's degree preferred.
  • Minimum of three years of experience in healthcare, behavioral health, public health, or youth services.
  • At least two years of program management or supervisory experience preferred.
  • Experience working with adolescents and transition-age youth.
  • Knowledge of integrated care models, care coordination, and social determinants of health.
  • Experience with electronic health records and healthcare quality reporting.
  • Strong leadership, communication, and organizational skills.
