Job description

Case Manager

Our company operates Residential Treatment Programs for individuals with Substance Abuse Disorder. We strive not only to restore sobriety but also to transform our patients' worlds from a state of everlasting darkness to vibrant technicolor. We believe that treatment is local, individualized, holistic, and relational. Currently, we have a presence in several states and are rapidly expanding nationwide.

Summary
The Case Manager acts as a member of a multidisciplinary team to assess and coordinate patients’ care among multiple service providers during inpatient and outpatient treatment.

Responsibilities

  • Assess patient out-of-facility biopsychosocial stressors (i.e. housing, financial, legal, medical, employment, etc.) and develop a strategy to meet these needs during and after treatment

  • Facilitate multiple care aspects including case coordination, information sharing, aftercare planning, and court-related issues

  • Maintain positive working relationships with support networks, government and community resources, employers, and referral sources

  • Complete all necessary documentation related to FMLA and Short-Term Disability

  • Maintain patient records by reviewing and completing case notes

  • Prepare patients for discharge by coordinating necessary community services to support sobriety post-treatment

  • Participate in weekly multidisciplinary treatment team meetings to assess patient needs and discharge planning

  • Participate in daily census management including coordination of discharge schedule and communication to the treatment and administrative teams

Qualifications

  • Bachelor’s Degree or higher in a behavioral science preferred, or a CADC/TCADC with a Bachelor’s Degree

  • Previous Targeted Case Management experience or supervision of the service since 2014

  • Valid, unrestricted driver’s license

  • Clean driving record for at least 3 years

  • 1 year of experience working directly with adults or adolescents in a human service setting

  • Must be committed to a drug-free lifestyle and responsible alcohol use

  • Personal passion or interest in substance abuse issues strongly preferred

  • Must meet all requirements within State Regulations
