We are looking for highly skilled, self-motivated individual who has experience with acute care hospital inpatient case management. This is a great opportunity to work for a large national hospital system that does not require nights, weekends, or holidays.

Requirements:

• One of the following licenses; RN, SW, LPN, LVN, or RT
• 2+ years clinical experience
• Previous hospital Case Management or discharge experience
