Job Description

Hospital Case Manager / Discharge Planner
This is a full time, fully benefited opportunity with the potential to go longer and even convert permanently. This is a national hospital system. We are looking for highly skilled, self-motivated, registered nurses (RN) who has experience with acute care hospital inpatient case management.

Requirements:

WI Registered Nurse (RN) license OR Licensed Social Worker (LSW)
2+ years clinical nursing experience
Previous hospital Case Management experience
