Registered Nurse ODA

Job Summary

As an ODA RN in Ohio, your role involves providing comprehensive healthcare services to older adults in the community. This includes conducting health assessments, developing care plans, administering medications, monitoring vital signs, and providing health education. You collaborate with interdisciplinary teams and community resources to ensure holistic care and support for older adults, promoting their well-being and quality of life as they age.

1. Health Assessment: Conduct thorough assessments of older adults' health status, including physical, mental, and emotional well-being. This involves reviewing medical history, performing physical examinations, and assessing cognitive function.

2. Care Planning: Develop individualized care plans based on assessment findings and in consultation with the patient, their family, and other healthcare professionals. These plans should address the specific needs and goals of each older adult, considering factors such as chronic conditions, mobility issues, and social support.

3. Medication Management: Administer medications as prescribed by healthcare providers, ensuring accuracy and adherence to medication protocols. Educate patients and caregivers on medication usage, potential side effects, and the importance of medication compliance.

4. Vital Sign Monitoring: Regularly monitor vital signs such as blood pressure, heart rate, respiratory rate, and temperature. Recognize and respond promptly to any abnormalities or changes in vital signs, taking appropriate action as needed.

5. Health Education: Provide education to older adults and their families on various health topics, including disease prevention, nutrition, exercise, medication management, and self-care strategies. Empower patients to take an active role in managing their health and making informed healthcare decisions.

6. Collaboration: Collaborate with interdisciplinary teams, including physicians, nurse practitioners, social workers, and therapists, to coordinate and optimize patient care. Communicate effectively with team members to ensure continuity of care and comprehensive support for older adults.

7. Community Outreach: Engage with community resources and organizations to facilitate access to healthcare services, support programs, and social activities for older adults. Participate in community health events, workshops, and screenings to promote healthy aging and preventive care.

8. Documentation and Reporting: Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and outcomes in electronic health records (EHR) systems. Prepare reports and communicate patient progress or concerns to relevant stakeholders as necessary.

Qualifications

·      Bachelors

·      Must have an active RN or LPN license

·      At least two years’ experience in the nursing field of mental illness

·      Must have a valid Ohio driver’s license

·      Current credentials according to the matrix in the Ohio Revised Code 5122-29.

·      Professional license required.
