Aegis Home Health and Hospice is currently seeking a compassionate Medical Social Worker (MSW) that has a passion for providing outstanding services to the elderly in Senior Living Communities and in-home health settings. This is a full-time position. The two offices are located in Anchorage and Wasilla.

Each Medical Social Worker (MSW) is an important part of the Aegis Health Care team. Aegis specializes in providing care for hospice and home health patients in senior living communities or in the patient's home. Job Responsibilities:

• Provide short-term individual counseling
• Assess the psychosocial status of patient/caregiver related to terminal illness and communicate findings to the RN and other members of the interdisciplinary group

• Complete social evaluations including family dynamics, caregiver abilities, communication patterns, risk for suicide, neglect or abuse, and plan intervention based on evaluation findings
• Address patient/family questions and issues
• Educate patient/family on and assist in preparation of advanced directives
• Maintain a collaborative relationship with personnel to support patient care
• Serve as a liaison between patient/caregiver and community agencies
• Maintain clinical records for all patients
• Active participation in team meetings
• Demonstrate flexibility with job duties and assignments

Skills & Qualifications:

• Master's degree from a school of social work (MSW) accredited by the council of social work education
• Minimum of one year experience in a health care setting; home care experience preferred
• Understand hospice philosophy, and issues of death/dying
• Strong communication and interpersonal skills
• Adaptive problem-solving skills
• Compassionate and caring attitude
• Valid driver's license, clean driving record, and reliable automobile
• Proof of current auto insurance
• Criminal background check clearance/fingerprint clearance
• Current CPR certification
• Negative TB test

Working Environment:

• Works indoors in Agency office and patient homes and travels to/from patient homes.
