Registered Nurse Case Manager Compensation

• The pay for this position is $40 - $45 per Hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Registered Nurse Case Manager Highlights

• The required availability for this position is Monday – Friday from 8AM – 5PM PST

Registered Nurse Case Manager Responsibilities

• Care Manager would be responsible to complete 6 assessments per week with our members in their home.

• Our position is with the Assessment Team and we are the ones to complete initial assessments for new members and we also complete reassessments for Level 3 members (low acuity).

• Our team also completes Medically Dependent Children’s Program Interest List assessments.

• Managing a case load for healthcare members

• Building care plans, medication review, assessments care plans

Registered Nurse Case Manager Requirements

• TX RN License
• Case Management experience
• Experience with electronic medical records
• Attendance is mandatory for the first 90 days

Registered Nurse Case Manager Preferences

• Bachelor’s Degree
• Home Health experience
• Long Term Care experience
