Job description

Job Summary- Community Missions has various Case Manager positions throughout the agency. The positions are responsible for providing comprehensive case management services to the clients served. This involves assessing client needs, developing and implementing care plans, connecting clients with community resources, and advocating on their behalf. Case Managers work closely with clients, their families, and a network of service providers to ensure the clients receive the necessary support to achieve their personal goals.

Key Responsibilities-

· Conduct assessments of clients’ strengths and needs.

· Develop and implement individualized care plans that address client needs and goals

· Coordinate with internal and external resources to connect clients with necessary services, such as housing, healthcare, employment, and education.

· Monitor client progress through regular follow-ups and adjust care plans as needed

· Advocate for clients to ensure they receive appropriate services and support.

· Maintain accurate and up-to-date case files and documentation in compliance with regulatory requirements and policies.

· Provide crisis intervention and support to clients in emergency situations

· Participate in community outreach and networking activities to build relationships with service providers and other stakeholders

· Ensure compliance with all relevant policies, procedures, and regulations

· On-call responsibilities in rotation with a team (for some positions)

Qualifications-

· Bachelors degree in a health service related field plus (Human Services, Criminal Justice, Psychology) 1 year professional experience in the human services field

· Valid New York State Driver’s License with clean driving record

· First Aid/CPR trained (agency will provide)

Positions Available:

· Community-Based Residential Case Manager ($39,000-$43,000)

o Provide Case management to ex offenders enrolled as participants in the Community Based Residential Programs

· Rapid Re-Housing Case Manager- Adult ($41,000-$46,500)

o Provide case management to help individuals and families move from homelessness to permanent housing with a focus on long-term stability

· Rapid Re-Housing Case Manager- Young Adults ($39,000-$43,000)

o Provide case management to help individuals (young adults) move from homelessness to permanent housing with a focus on long-term stability

· Supportive Housing Case Manager ($39,000-$43,000)

o Provide case management of individual tenants of our Supportive Housing Program and providing restorative services.
