Your Job Opportunity


Case Managers support clients with mental illness with daily living activities to increase their level of functioning, reduce hospitalizations and ensure their access to community resources. ย 


Provide ongoing linkage, monitoring, planning, evaluation and advocacy for clients within the medical and social service community, law enforcement and health department. ย Safely transport to related appointments.


Teach and support daily living activities such as: shopping, nutritious meal preparation and budgeting of financial resources.


Monitor symptoms, de-escalate behaviors and respond to crisis involving medical and community professionals as needed.


Requirements

Qualifications Needed for Success for a Case Manager


Associate's Degree in social work or a relegated field with a minimum of one year of direct contact care in mental health preferred. ย 

Working knowledge of mental illness, psychiatric symptoms, and their impact on functioning and behavior.

Ability to meet direct client contact goals including providing required documentation such as service notes, needs assessments and individual service plans in the Electronic Medical Records system. ย 

Valid driver's license, a reliable vehicle & insurance with a good driving record. ย Ability to safely transport clients.

Ability to pass a pre-employment background check and drug screen.
