Intake & Lead Functional Screener, CCS

GENERAL SUMMARY:
Comprehensive Community Services (CCS) is a voluntary program that provides support and services for adults who are coping with mental health or substance abuse diagnosis. CCS is a community-based program in which the majority of services are provided in homes & communities. The program is person-centered & uses client-directed service plans to describe the individualized services that will support the consumer to achieve their recovery goals. Services are provided by teams of professionals, peer specialists, & natural supports, all coordinated by a CCS Care Coordinator.
The Intake Coordinator is responsible for ensuring all required initial paperwork for any new referral is submitted within a timely manner according to DHS36.
ESSENTIAL FUNCTIONS:
- Gather information to ensure eligibility, communicate program components to client and families.
- Establish and maintain current computerized and manual systems to track intake and referral of family.
- Coordinate in person meetings to complete the initial application and agreement and complete paperwork.
- Consult with Supervisory staff/MHP to determine most appropriate CCS Care Coordinator for the consumer
- Complete Functional Screenings courses, already certified preferred
- Coordinate or recommend in-home services, community resources & informal supports.
- Submit appropriate documentation to all parties involved
- Work closely and cooperatively with any external parties involved.
- Attend weekly staffing, trainings, meetings and clinical consultations as required
- Is available during program hours to assist families with crisis intervention.
- Adhere to St. Charles Youth & Family Services Policies and Procedures.
- Support the goals of the Agency Mission / Vision.
- Other duties as assigned.
MINIMUM SKILLS, KNOWLEDGE, AND ABILITIES REQUIRED:
- Bachelor Degree in Social Work or related Human Service field, Master’s preferred
- Current CCS experience preferred.
- Bilingual preferably Spanish
- Proficient verbal communication and formal report writing.
- Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
- Ability to familiarize self with local community resources.
- Knowledge of DHS36 policies and procedures
- Knowledge of methods of evaluation, treatment and ability to interpret written results.
- General advocacy and counseling principles related to alcohol and drug abuse/addiction, human growth and development and human sexuality.
- Ability to understand and apply agency approved crisis management techniques.
- Ability to coordinate and conduct individual, family and group meetings.
- Attention to detail and good organization skills.
- WI Drivers License and valid insurance required.
- In-home and community service delivery
- Travel approximately 70% using a company vehicle.
- Potential for exposure to blood products, contagious disease etc.
