SUMMARY:

Native American Community Clinic’s Medication-Assisted Treatment (MAT) Program is seeking an experienced Registered Nurse. This full-time position will help to support the direct-prescribe program, which is integrated into the primary care clinic, as well as the daily dosing program in which patients present to the clinic daily for medication administration.

The program uses an integrated, harm reduction approach to providing holistic care to patients with Opioid Use Disorder. The program is culturally specific but does welcome all patients. The integrated team includes but is not limited to Doctors, Mental Health Professionals, Alcohol and Drug Counselors, Spiritual Care Specialists, Nurse Practitioners, and Peer Recovery Specialists.

Our mission is to promote the health & wellness of mind, body, and spirit of Native American families. DUTIES AND RESPONSIBILITIES:

  • Perform nursing duties such as administer medications, collect vitals, assist with procedures, provide patient education
  • Work with providers to complete Nurse Assisted patient visits
  • Facilitate coordination with providers to manage relationships with outside providers, facilities, transitions-in-care, and referrals
  • Work in collaboration with patients and caregivers to promote timely access to appropriate care, increase utilization of preventative care, and reduce emergency room utilization and hospital readmission
  • Promote patient-centered care by assessing unmet health and social needs
  • Maintain accurate medication dispensing and inventory records
  • Collect urine specimens and perform breathalyzer tests
  • Assist with tracking and review of lab results
  • Complete charting and billing documentation in a timely fashion
  • Provide patients with safer use supplies such as syringes and smoking supplies
  • Collect and maintain data for departmental and/or grant reporting
  • Attend regular meetings with providers, care coordinators and medical teams
  • Participate in work groups as requested
  • Other duties as assigned

SUPERVISORY RESPONSIBILITIES:

  • There are no supervisory responsibilities for this position

QUALIFICATIONS:

  • Education and/or experience: Minimum 2 year experience providing direct patient care services in clinic setting, community clinic preferred; previous experience working with Native Americans or other marginalized communities preferred. Knowledge and understanding of the principles of harm reduction.
  • Certificates, licenses, and registrations required: Registered Nurse (RN) with active, unrestricted license to practice in Minnesota; BLS CPR, First Aid or other needed certifications when requested; proof of current COVID-19 vaccination.
  • Computer skills required: Internet Software; Spreadsheet Software (Excel); Word Processing Software (Word); Electronic Mail Software (Outlook); Presentation software (PowerPoint); experience using EMR preferred.
  • Other skills required: Works well independently and on a team; excellent communication, customer service, and case management skills.
