Registered Nurse Case Manager Hospice

Constellation Health Services is seeking a Registered Nurse (RN) Case Manager for our hospice program!

Schedule: Monday-Friday 8:30am-5:00pm.

Territory: This position will cover the Berwick, Kennebunk, Wells and surrounding areas.

Constellation is seeking compassionate, skilled, experienced hospice care and/or community field Registered Nurse to work with our hospice program in Maine. We know that RNs who want to work in hospice care have a heart for patients at this important time in their lives and have a passion for supporting them and their families through their life transitions.

This is You:

• Graduate of an accredited RN program
• Registered Nurse Licensed in the state of Maine
• In-home Hospice or community care field experience
• Previous direct hospice patient care experience is strongly desired
• Must be able to travel within an assigned territory and possess basic computer skills

Responsibilities:

• Provides skilled nursing care including the administration of medications and treatments as prescribed in the physician's plan of care for patients in a home setting.
• Performs a comprehensive assessment and coordinates the plan of care with all disciplines associated with the patient's needs, including family and primary caregivers.
• Observes, evaluates and reports on the patient's physical and emotional status to the nursing supervisor and the attending physician whenever a change occurs.
• Participates in weekly Interdisciplinary Team Meetings, providing updates on patients' physical, emotional, psychosocial, and spiritual status.
• Attends all required staff meetings to enhance professional development.
• Maintains and processes all documentation in compliance with agency standards.
• Will utilize a computer-based documentation system and submit all required clinical documentation, time records and other required information in the required time frame.
• Instructs, orients and supervises other clinical personnel as necessary.
• Provides input to Home Health Aide Supervisor as needed for the purpose of hospice aide evaluation.
• May act as a liaison with institutions to promote continuity of care as the patient moves from lone evel of care to another.
• May participate in community activities as a representative of the agency.
• Participates in weekend rotation and on-call rotation as needed.
• May perform other duties as assigned.
