Woodfords Family Services is growing and searching for a full-time (32-40 hrs. per week) Transition to Adulthood Coordinator! This is a unique role that supports youth with disabilities prepare and process through their transition into young adults.

The Coordinator will provide services in and throughout York and Cumberland County


Position Summary

The Transition Coordinator will be responsible for working with adolescent youth, ages 16-20 years old, and their families to provide support, advocacy, and linkage to services while encouraging independence, family empowerment and self-sufficiency. The coordinator will assist consumers and families in developing goals and future planning throughout the transition to adulthood.

The Transition Coordinator serves as a liaison between other agencies and services involved (public schools, behavioral health agencies, consultants, etc.) to ensure all areas of development are maintained, and offers additional resources when necessary to promote a successful and supportive transition of services.


What we're looking for

Full-time, 32-40 hours per week

Monday-Friday; 8:00am-4:00pm

$42,500-$46,000 annual salary (commensurate with experience)

$1,500 Sign-on Bonus! - Eligible at 6 months of employment.

Qualifications

  1. Bachelors Degree in Social Work or related Human Service field (LSW Preferred).
  2. At least (3) three years experience in the field.
  3. Knowledgeable about regulatory guidelines.
  4. Possesses knowledge of developmental disabilities, mental health diagnosis, and community resources.
  5. Valid driver's license and have use of a vehicle.
