Title: Home Health Aide (HHA)
Department: Resident Care
Qualifications: 1. Eighteen (18) years of age required; high school diploma or
equivalent preferred.
2. Certification by an approved basic training program in home
health aide services approved by the Department of Health and
possess written evidence of such certification. Must maintain
current annual skills testing approval.
3. Must have proof of Immunizations. 2 MMR or titer. Submit to TST at time of hire and annually.
4. Submit to annual flu or use of mask during the flu season.
5. Successful clearance of a Criminal History Background Check.
Job Duties:
Position Duties include but are not limited to:
- Responsible for responding immediately to residents’ requests. Timely response to resident pull cords.
- Assist residents with the following Activities of Daily Living (ADLs), according to their plan of care, individual needs, and care preferences:
- Bathing
- Dressing
- Eating
- Exercise
- Housekeeping
- Assisting with Medical Supplies/Equipment
- Positioning
- Ambulation
- Grooming
- Toileting
- Transferring
- Walking
- Personal Emergency Response
- Routine Skin Care
- Laundering of Clothing
2. Notify Supervisor of changes in residents’ conditions or needs:
- Receive report about status of residents from nurse prior to providing care.
- Follow plan of care in providing daily living and personal care services for resident.
- Inform nurse after providing personal care services, taking of vital signs, weighing resident, and other services or tasks, as assigned.
3. Maintain facility standards for resident care by:
- Maintain knowledge of protecting rights of residents.
- Ensuring that all residents are treated with respect and kindness.
- Reporting to nurse in charge about facility equipment that needs repair, unsafe conditions in room or on floor, or personal needs of residents. Make sure all equipment is clean, stored appropriately and in good working order.
- Attending at least twelve (12) hours of in-service programs annually to reinforce or enrich knowledge of resident needs and care.
- Showing respect for personal possessions of residents and helping maintain an orderly pleasant environment on the floor for staff and residents.
4. Assist with service in the dining room for all meals as needed. Maintain facility standards for service and customer focus techniques.
5. Inform nurse responsible for the care of resident and document duties performed as necessary before leaving for break or end-of-shift.
6. Assist LPN/Resident by providing Medication reminders to Residents on assignment.
7. May assist Resident on assignment with Oxygen by providing reminders or guidance with tubing but unable to set liter flow.
8. Arrives to work on time, regularly, and work as scheduled, responsible for own transportation.
Must be Available weekends and Holidays.
