MUST HAVE HHA/PCA CERTIFICATE

We have a female patient that needs service 8am-10pm 3 days a week.

Member has no pets, doesn't smoke.

HHA/PCA Certificate

Unexpired Identification (Passport, Permanent Resident Card, NYS ID, etc)

Social Security Card

Pre-Employment physical (within 6 months old)

Rubella and Rubeola Immunization.

Two professional References.
