Amber Court of Westbury is seeking a dynamic HHA Coordinator.

The HHA Coordinator manages home health aide services by scheduling, training, and supervising staff to ensure high-quality resident care. They coordinate care plans, oversee daily operations, ensure compliance with regulations, and serve as a liaison between residents, families, and the healthcare team.

Key Responsibilities:

  • Staff Scheduling & Management: Scheduling HHAs for shifts, managing coverage, and onboarding new staff.

  • Care Coordination: Implementing Care Plans and Individualized Service Plans (ISPs) while monitoring residents' care needs.

  • Supervision: Monitoring the performance of aides, providing coaching, and ensuring adherence to safety and health protocols.

  • Quality Assurance: Maintaining compliance with state regulations, reviewing daily logs, and reporting changes in resident conditions.

  • Communication: Acting as a primary contact for direct care staff and facilitating daily team meetings.

Qualifications:
