Job Description

We are seeking a talented individual to join our team! PSG/CIP is a multi-service community-based social services organization offering mental health and programming throughout eastern Wisconsin.

As our new Care Coordinator, you will be responsible for providing care coordination services to individuals with significant mental health and substance abuse diagnoses. Care coordination services incorporate the wraparound model of service delivery which supports in the home, community, and other areas of the individual's life and encourages a team approach to assist in meeting their needs. Care coordination will include a strength-based recovery and person-centered approach to service delivery.

KEY RESPONSIBLITIES OF THE CARE COORDINATOR:

  • Interview and evaluate clients and families to complete intake and assessment process (PPS/NOMS and Functional Screen).
  • Provide services for an ongoing caseload of clients consistent with DHS 36 regulations.
  • Complete a thorough Medical Assistance (MA) compliant Strengths and Needs assessment as necessary.
  • Analyze client situations, capabilities and problems to determine services required to meet treatment needs of the individual.
  • Develop, implement, evaluate and revise goal oriented and strength-based client Recovery Plan of Care (RPOC) and update as changes occur.
  • Facilitate team meetings with service providers and supports, and monitor progress of goals and services.
  • Develop client crisis/safety plans.
  • Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
  • Develop a support team comprised of formal and informal supports.
  • Attend client/family court proceedings, psychiatry appointments, or other appointments which may require support or advocacy.
  • Receive clinical supervision and implement recommendations into treatment plan.
  • Safely transport clients as needed.

OFFICE LOCATION & HOURS: Hybrid model of time spent out in the community completing home visits with clients, with some time spent at the office in West Allis, WI and flexibility to work remote. Hours will vary, but typically Monday-Friday between 9:00am to 5:00pm.

SALARY: $20.25-$21.50 per hour for candidates with a Bachelor's Degree

$25.00 per hour for candidates with a Master's Degree

REQUIREMENTS:

  • Must possess at least a Bachelor's degree in Social Work, Psychology, Criminal Justice, or other related field.
  • Must have reliable transportation on a daily basis, a valid driver's license and automobile insurance..
  • One year of experience working with clients with significant mental health or substance abuse diagnoses is preferred.
