Summary Description:


Assist agency RN/Healthcare Supervisor  (HCS) in overseeing health and safety supports for all individuals with a focus on individuals in our Residential Program. Assist in completing all necessary medical documentation, developing residential policies and procedures, trainings for program staff, and hospital discharges. Support residential managers with clinical questions both in person and via the phone.


The LPN would work 3 days a week in the day program and would focus on the care of participants who are also in our residential program. These 3 days the hours would be 8:30 - 3:30. The other 2 days would be from 9:30-5:30 with the expectation being that they would be at the residences from 3-5:30pm on those days. There is some flexibility on the two late days, but not a Monday or a Friday.


Requirements:          


Graduate of an accredited LPN program. Current Massachusetts license as a Licensed Practical Nurse in good standing. Ability to lift and support body weight of any program individual with assistance of another employee. Professional appearance and demeanor.  Possession of a valid Driver’s license, a clean driving record and a willingness to use agency vehicle in the course of employment.


Preference:               


Experience with individuals with developmental disabilities.  Training/proficiency in MAP and Universal Precautions.


Duties and Responsibilities:

 

  1. Provide healthcare services for all program recipients with a focus on residential program recipients.
  2. Assist with all medical aspects of a program recipient’s admission process.
  3. Assist with coordinating medical services with each recipient’s primary physician when needed.
  4. Participate as a member of an inter-disciplinary evaluation team.
  5. Participates in ongoing interdisciplinary evaluations and review of each recipient’s needs and status (as assigned).            
  6. Assist HCS with maintaining currency of records on an ongoing basis.
  7. Maintain medical records, health plans, medical recommendations/authorizations and medication information for each program recipient as it pertains to the day habilitation service plans.
  8. Obtain reports and approval of treatment plans from primary physicians; and ensure that records/documentation meets regulatory requirements. Meet or exceed regulatory requirements for CARF and Day Habilitation programming.
  9. Assist with updating monthly progress notes and semi-annual reviews.
  10. Assist with completing annual nursing assessments for recipients’ of Day Habilitation Service Plan.
  11. Assist with providing training and education to staff and individuals in relevant health care areas, including Safety, Universal Precautions, and others as requested by the Program Director or DON.
  12. Participate in recipients’ day habilitation and residential service planning and other team meetings, as requested.
  13. Effectively communicate with state agency personnel, other service professionals, family members, guardians, etc.
  14. Provide on-site assessment and treatment of individuals in the day programs and in the residential homes, as needed.
  15. Administer medications and provide medical interventions as directed by physician order.
  16. Follow all regulatory requirements re: medication administration, security and storage.
  17. Provide assessment of individuals for self-medication (as needed).
  18. Provide consultation/staff training as requested by HCS or DON.
  19. Assist program staff in providing direct care/personal care supports to individuals.
  20. Support the auditing of medication and healthcare records on a regular basis.
  21. Support program participants’ right to express their religious affiliations and cultural traditions through preferred and relevant activities.  Support diversity of individuals served through respect of religious preferences and/or cultural backgrounds.
  22. Perform other duties as assigned.
