JOB SUMMARY

The nurse manager functions as the director of nursing services and is responsible for implementing a system of nursing care that meets the needs of the resident population in the Skilled Nursing Facility (SNF). She or he oversees and monitors resident care to ensure it meets professional nursing practice standards, and District of Columbia/Federal regulations.

 

JOB FUNCTIONS

1. Administrative Functions:

· Develop and implement an organized nursing system for the delivery of care and services.

· Coordinate nursing services with other disciplines and departments. (I.e. medical staff, social services, respiratory services, admissions, pharmacy, rehabilitation, dietary, housekeeping, laundry, social services, laboratory, maintenance, etc.)

· Work with other management personnel at the client’s health facility to facilitate delivery of nursing services and other services.

· Utilize the expertise of other departments and services to address nursing issues in facilitating delivery of resident services.

· Develop the nursing budget for the nursing department to include hours of care per day per patient, staffing mix to deliver hours of care, necessary supplies/equipment and technological resources.

· Operate nursing department within budgetary constraints.

· Develop/review policies/procedures due to regulatory changes.

· Actively participate in Quality Assurance Performance Improvement Committee (QAPI) and activities. Responsible for gathering data, analyzing data, implementing plan of corrections, and reporting to the QAPI Committee.

· Monitor Care and document findings, develop plan of corrections, and report findings to the Quality Assurance Performance Improvement Committee.

· Coordinate, complete, and transmit the Minimum Data Set (MDS) as required by regulation.

· Provide interdisciplinary team with MDS schedule and participate in care planning meetings with resident and his/her responsible agent.

· Ensure that nursing staffing levels meet resident needs and regulatory requirements including standards as defined by the SNF’s Operation Plan for ventilator beds.

· Participate in marketing events to promote SNF and grow census.

· Participate in professional associations to represent SNF, and gain knowledge as to regulatory changes and care trends.

· Collaborate with support services such as security, engineering, and others to ensure the safety and security of residents and staff.

· Maintain 24/7 accountability for nursing care.

· Conduct investigations for allegations of abuse and report same as required by regulations in coordination with NHA.

2. Admission, Transfer, and discharge functions:

· Review referrals for appropriateness in conjunction with other disciplines to ensure the SNF is able to meet the resident’s needs.

· Coordinate availability of equipment, medications, and other supplies per the resident’s needs.

· Monitor through daily clinical meeting proper documentation for admissions, transfers, and discharges.

· Meet and greet residents and their responsible agents within 72 hours of admission.

· Monitor and evaluate transfers to higher levels of care for appropriateness.

· Monitor and evaluate discharges for appropriateness and with the goal of reducing re-hospitalizations.

· Review transfers to higher levels of care and discharge concerns with Medical Director and NHA.

3. Personnel Function:

· Provide oversight to nursing staff who provide nursing care to the facility’s residents.

· Ensure that the nursing staff have updated licenses, CPR certification, and other credentials as required.

· Conduct monthly meetings or more frequently if necessary with nursing staff to maintain open lines of communication.

· Evaluate nursing staff competencies and provide training/educational opportunities for skill improvement or maintenance.

· Provide training/in-services to staff based on identified needs, or coordinate the provision of the latter by appropriate personnel/consultant/outside agency.

· Complete performance evaluation on a timely basis.

· Develop and implement programs to improve morale level amongst nursing personnel.

· Monitor nursing staff time and attendance to address tardiness and absenteeism.

· Evaluate performance and counsel/discipline according to applicable policy and in collaboration with Administrator (NHA) and HSC Human Resources.

· Participate with the recruitment of staff (i.e. interview and hire nursing staff and other disciplines as assigned).

 

4. Staff Development:

· Participate in at least 12 hours of continuing education programs relating to administration, pediatric care, or other related areas of care as required by regulation.

  • Attend and participate in scheduled training, educational classes, orientation programs and activities.
  • Attend workshops and other educations programs to understand regulatory trends/changes and clinical advances.

· Conduct or coordinate in -service training as needed for nursing staff and other disciplines as needed.

5. Resident Rights:

  • Maintain the confidentiality of all resident care information.
  • Ensure that all nursing care is provided in privacy.
  • Knock before entering the resident room at all times.
  • Make sure that grievances and complaints are addressed/documented and provided to Social Worker.
  • Address each resident by preferred name.
  • Acknowledge the resident right to be free of verbal, mental, sexual or physical abuse and involuntary seclusion.
  • Explain to the resident each activity to be undertaken prior to the activity.
  • Be an advocate for the resident. Greet friends and family and address concerns.
  • Encourage resident participation in care activities and making choices.
  • Ensure that each resident is dressed to enhance individual’s self-esteem their preference, age, and life style.
  • Residents/families have a right to report problems with staff without retaliation.
  • Answer all call lights promptly and assist residents as needed and requested by family or other staff members.
  • Residents have a right to keep their personal belongings. When a resident expresses concern that they are missing personal property, make sure it is documented/investigated and provided to the Social Worker.
  • Residents will be treated in a manner which maintains and enhances their dignity.

QUALIFICATIONS

Required Experience:

1. Must have a minimum of two (2) years management experience.

2. Competent in dealing with ventilator dependent population.

3. Experience in working with children and young adults with specials preferred.

License/Certifications:

1. Current RN license in the District of Columbia

2. CPR certification

Required Education:

Bachelor’s degree in nursing preferred; or have three years of clinical experience in appropriate nursing specialties. (i.e. ventilator care, pediatric care)

SUPERVISORY RESPONSIBILITIES

The incumbent will be responsible for the general direction of 40-50 staff members.
