NEW Care, Inc. is currently seeking a Dietician Consultant to work with our Dietary Services Department.  We are looking for an individual that is dependable and energetic to join our excellent team of dietary personnel.  This is a contracted position providing services and education to residents and staff of NEW Care, Inc.'s skilled nursing facility and assisted living facility, that supports and maximizes the resident's nutritional well-being.  This position reports to the Dietary Manager.  The consultant will work approximately 10 hours/month, at an hourly rate of $50.00 to $55.00/hour depending on experience.  In addition, the individual selected will work remotely with an onsite visit quarterly.  Come join our team!

Responsibilities:

• Evaluates, develops and completes comprehensive nutrition assessment and reassessment consistent with CMS regulations.
• Assesses on-going nutritional status to determine appropriate nutrition intervention(s) and recommendations.  Sends recommendations to the Dietary Manager.
• Identifies and develops the treatment and nutritional goals for POC.
• Provides nutrition education and counseling based on individualized patient needs as requested.
• Documents nutrition progress upon admission, quarterly, annually, and with significant change of condition per requested.
• Collaborates with Dietary Manager to facilitate adequate and appropriate nutritional care.
• Reviews menus quarterly with Dietary Manager, addressing nutritional content, palatability, and presentation.
• Reviews residents who have wounds or significant weight loss for nutritional needs as needed.
• Presents nutrition related in-service for caregivers as needed, but no more than once annually.
• Other duties as assigned.

Qualifications:

• Master's or higher degree granted by a regionally accredited college or university in the United States with completion of the academic requirements of a program in nutrition or dietetics accredited by an appropriate national accreditation organization recognized for this purpose.
• Has completed at least 900 hours of supervised dietetics practice under the supervision of a registered dietitian or nutrition professional.
• Is licensed or certified as a dietitian or nutrition professional by the State of Wisconsin.
