Job Description:
We are currently seeking an Experienced Home Health Nurse with a minimum of 1 year of home health nursing experience. Proficiency in Axxess software and exceptional charting skills are essential for this role. As a crucial member of our team, you will play a vital role in delivering top-notch care and maintaining accurate documentation.

Key Responsibilities:

• Perform comprehensive health assessments in clients' homes.
• Develop personalized care plans to achieve optimal outcomes.
• Administer medications and skilled nursing care as required.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and caregivers on relevant health topics.
• Maintain precise electronic records using Axxess software.

• Ensure compliance with all home health regulations.

Qualifications:

• Minimum 1 year of home health nursing experience.
• Proficiency in Axxess software with at least one year of hands-on experience.
• Strong charting and documentation skills.
• Current nursing license in Illinois.
• Excellent communication and interpersonal abilities.

• Reliable transportation and valid driver's license.
