Job Description

NURSE PRACTITIONER /HYBRID REMOTE
Pleasant Prairie, WI
105-135K + Bonus

Reports to a Director for administrative supervision and the Practice Site Physician or designee for clinical supervision. Is responsible for the coordination and delivery of primary care, as specified by the appropriate certifying agency, to patients of specific area employers. Provides clinical and administrative assistance to programs, etc. Works collaboratively to meet clinic objectives for patients and clients.

Job Responsibility

Coordinates and delivers primary health care and wellness care by:

  • Working within the approved scope of practice as designated for this type of professional.
  • Obtaining complete medical history and physical data on patients.
  • Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures as needed.
  • Performing a complete physical exam and recording findings. Managing acute and chronic medical needs of patients.
  • Ordering appropriate laboratory and diagnostic procedures.
  • Synthesizing data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.
  • Administering, dispensing, and prescribing medications and injections.
  • Interviewing and advising patients regarding health and illness prevention. Recommending community resources to meet patient and family needs.
  • Instructing patient and family regarding medications and treatment instructions.
  • Maintaining and reviewing patient records, charts, and other pertinent information. Posting tests and examination results.
  • Triaging patient telephone calls and providing consultation.
  • Providing monitoring and continuity of care as appropriate between visits.
  • Collaborating with a Medical Physician in accordance with professional guidelines.

Provides clinical and administrative assistance, e.g., education, follow-up, and the like by:

  • Participating in health education.
  • Maintaining related records and statistical data and preparing statistical reports.
  • Participating in interdisciplinary care conferences and assisting with referrals and dissemination of information to appropriate care partners.
  • Assisting in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes.

Performs other functions to maintain personal competence and contribute to the overall effectiveness of the clinic by:

  • Developing and maintaining affiliation with professional and community organizations.
  • Assisting in development and compliance of clinical protocols, policies and procedures.
  • Participating in quality review and improvement programs; monitoring quality and outcomes of care rendered.
  • Performing other job related assignments and special projects as directed.

ORGANIZATIONAL RESPONSIBILITIES

Associate complies with the following organizational requirements:

  • Attends and participates in department meetings and is accountable for all information shared.
  • Completes mandatory education, annual competencies and department specific education within established timeframes.
  • Completes annual employee health requirements within established timeframes.
  • Maintains license/certification, registration in good standing throughout fiscal year.
  • Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
  • Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self.
  • Adheres to regulatory agency requirements, survey process and compliance.
  • Complies with established organization and department policies.
  • Available to work overtime in addition to working additional or other shifts and schedules when required.

Requirements:
-Bachelor's degree in Nursing required, Master's of Science Degree in Nursing is preferred.
-A valid and current RN license in the state of Wisconsin
-The successful completion of a nurse practitioner program with applicable national certification.
- Attainment of prescriptive authority or successful completion of applicable course work and pass state board exam to obtain prescriptive authority within six months of hire date.

Preferred

Bilingual Spanish speaking and/or writing skills desired.Job Qualifications

Education and Experience

The knowledge, skills and abilities as indicated below are normally acquired through the successful completion of a Bachelor's degree in Nursing. A valid and current RN license in the state of Indiana is required. The successful completion of a nurse practitioner program with applicable national certification is required. Two years of clinical nursing experience in practice area and successful completion of a Nurse Practitioner six month internship for new graduates or comparable experience is required. Master's of Science Degree in Nursing is preferred. Attainment of prescriptive authority or successful completion of applicable course work and pass state board exam to obtain prescriptive authority within six months of hire date. Bilingual speaking and/or writing skills are a plus.

Knowledge & Skills

  • Requires a thorough knowledge of practice standards and policies and procedures of MMG and the appropriate professional organization.
  • Requires a high level of analytical skills necessary to assess the patient and participate in follow-up care planning.
  • Demonstrates interpersonal skills necessary to effectively interact with patient(s), family of patient(s), physicians, other professionals and paraprofessionals.
  • Demonstrates high level of communication skills, both oral and written, necessary for effective communication with patient/family, physicians, and referral sources.

Job Types: Full-time, Part-time

Salary: $105,831.32 - $135,274.69 per year
