Requirements

 We are seeking a qualified professional to provide Case Management for Veterans to assist with finding suitable housing. This position is eligible for employee benefits, including vacation/sick leave accrual, 10 holidays each year, health benefits, 401(k), and much more! Our office hours are Monday - Thursday, 8:00 am to 5:30 pm and our offices are closed on Friday - giving you a three-day weekend! The salary range for this position begins at $37,000 and goes to $46,000 annually. 


Position Objective

This position is responsible for providing short-term Strengths Based Housing Case Management services for the Veterans Program, funded through the VA Supportive Services for Veteran Families.


Responsibilities

Contribute reports for SSVF, as requested.

Maintain a 90% score on internal clinical and administrative reviews.

Complete assessments and intakes for new SSVF enrollments as needed and coordinate with case managers daily. 

Enter client data and outcomes for SSVF into HMIS, ETO and Intacct as needed, such as progress notes in ETO to ensure client data is complete and updated. 

Maintain a file on each participant that includes eligibility documents, personal goal plan, case notes documenting action taken, progress, challenges, and follow up, and all required agency documents (e.g., Bill of Rights, Release of Information).

Maintain comprehensive statistical data on participants and complete other reports as requested.

Maintain accurate records of financial assistance provided to participants and progress with goals in HMIS and ETO.

Prepare case records for proper storage after participant exits program.

Develop and maintain a positive supportive relationship with grantors.

Attend agency and community meetings as requested.

Attending supervisory meetings.

Maintain working relationships with community agencies (e.g., VA, HMIS and Veteran Service Organizations, Salvation Army, Restart, Local CoC, coordinated grantee Mtgs.) to provide comprehensive service for participants and to stay abreast of current trends and resources available.

Attend in-service training and outside conferences/workshops.

Represent the agency's mission, philosophy, and function to the community.

Provide ideas for programming specific to educational and housing needed for participants.

Assess employment needs, make referral to appropriate resources which include agency workforce program and monitor employment success. 

Assess eligibility for veteran and mainstream benefits; make referrals to community partners, as needed, to secure eligible benefits.

Outreach and travel to the existing rural counties in the southern portion of Catholic Charities catchment area. Perform and track outreach in our 18-county catchment area. 

Conduct interviews with potential participants emphasizing their responsibility for participation in the strengths-based model of case management which includes personal goal plan with clients.

Schedule weekly sessions per month with participant who are homeless and monthly sessions for participants that are housed to develop and/or monitor progress with goals.

Provide advocacy services as needed (e.g., court appointments, landlord/tenant conflict resolution). Provide information, referrals, and supportive services as needed. 

Provide financial assistance in accordance with program policy and procedures.

Maintain contact with participants after completion for the purpose of follow up, outcome tracking and follow up services.

Perform other duties as assigned.
