Job Title: Licensed Social Worker LCSW PACE
Job Summary
The Clinical Social Worker is responsible for the direct social work and case management services to the participants of the MJH PACE Program. The Clinical Social Worker contributes the psychosocial perspective to the interdisciplinary evaluation, assessment, plan of care, ongoing services, and disenrollment process. Social Work interventions could include individual participant contacts, appropriate collateral contacts, participant and family education, assessments, counseling, mobilizing resources, addressing mental health needs as they arise, ongoing case management and disenrollment procedures.
Essential Job Functions
· Collaborates with the interdisciplinary team to ensure effective, efficient, and appropriate care to optimize the health status and quality of life of (PACE Program) participants
· Participates in the interdisciplinary team’s initial assessments, care planning and ongoing re-evaluations of participant’s case
· Refers participants and families to appropriate community agencies or facilities and acts as liaison with such organizations and as advocate for participants
· Encourages effective relationships among staff geared to the building and maintenance of cohesive team
· Consults and advises staff members as to the relationship of social, emotional, and cultural factors to health and medical care
· Educates staff members on the availability of social services in the community
· Participates in Program and Policy development of the Social Work component of Florida PACE Centers
· Completes Health record documentation according to established documentation standards and in a timely manner
· Continue professional development through attendance at staff meetings and engaging in educational opportunities to maintain professional competence and licensure
· Consistently follows the Social Work Code of Ethic, treats all people with respect, maintains confidentiality and strives toward service excellence
Job Requirements
Education:
· Master’s Degree from a school of Social Work accredited to the Council of Social Work Education
Experience:
· Two years’ experience in case management or social work in a hospital, nursing home, or community-based program
Licenses/Certifications:
· Licensed Clinical Social Worker required
Abilities required
· Excellent assessment and counseling skills
· Knowledgeable regarding social systems and institutions, and individual behavior
· Can skillfully apply appropriate interventions to meet the needs of the participant and family
· Thorough knowledge of psychosocial dynamics of individual & family
· Knowledge of geriatrics / gerontology preferred
· Excellent communication and interpersonal skills
· Must be computer literate
· Ability to work closely and well with others disciplines
· Bilingual in English/Spanish or English / Creole

Florida PACE Centers at Miami Jewish Health is one of the largest Programs for All-Inclusive Care for the Elderly (PACE) in the state of Florida. We currently serve over 900 participants spread over four locations: Hialeah, Westchester, Miramar, and our main campus in the Buena Vista neighborhood of Miami. PACE is a comprehensive, fully integrated, provider-based health program for those who require and a nursing home level of care. The PACE philosophy is centered on the belief that it is better for frail individuals and their families to be served in the community whenever possible. Although all PACE participants are eligible for nursing home care, 90 percent continue to live in the community.
