Hospice of the Piedmont is seeking a dedicated and compassionate full-time Clinical Services Manager (CSM) serving on the Rapidan team, which includes counties in the northern region of our territory, in and around Warrenton, VA. As CSM you will direct the care given by an interdisciplinary team of professionals and volunteers in providing services to hospice patients and their families. You will displace a commitment to excellence while supervising the team of Team Coordinator, CNA Coordinator, Social Worker(s), Chaplain, RN Case Managers, Licensed Practical Nurses, and Hospice Aides (CNAs).

Requirements:
  • Education/Licensure: Licensed as a Registered Nurse in the Commonwealth of Virginia.
  • Experience: 5 or more years of relevant clinical experience.
  • Knowledge: Demonstrates knowledge of EHR system and utilizes the system in the daily management and oversight of the IDT’s performance. A broad knowledge and experience in the delivery of hospice care. Knowledge of hospice regulations, policies and procedures, conditions of participation, and HIPAA compliance regulations.
  • Skills: Oral and Written Communication, Active Listening, Time Management, Leadership, Computer Literacy.
  • Mindset: Flexibility, ability to function in stressful situations, motivating others, commitment to excellence, and strong sense of integrity.

Responsibilities/General Duties (Including but not limited to):

  • Manages the delivery of all skilled and non-skilled services to patients and families ensuring quality patient care.
  • Leads the interdisciplinary team meeting.
  • Takes primary responsibility to assure that all necessary information is accurate and available for timely and accurate billing.
  • Takes primary responsibility to assure patients record information is accurate, current, and available according to regulations.
  • Ensures that services are provided according to hospice regulations, policies and procedures, conditions of participation, accreditation, and HIPAA compliance regulations.
  • Oversees all direct care costs, patient care vendors and ancillary services to ensure service delivery and quality including but not limited to medical supplies, staff mileage, DME, pharmacy, physical therapy, and dietetic services.
  • Role-models the mission, vision, and values of Hospice of the Piedmont.
  • Ensures that the patient and family are the center of every action and decision under a current plan of care.
  • Provides an atmosphere where the IDT continually strives to exceed the patient and family expectations.
  • Ensures superior customer service by providing prompt service recovery.
  • Address all complaints in accordance with HOP policies and procedures and/or legal compliance requirements.
  • Actively participates in the Quality Assessment Performance Improvement process.
  • Responsible for meeting/exceeding established quality goals.
  • Provides guidance and direction to IDT members and ensures they receive required and appropriate training including training staff to trainers for new employees.
  • Reviews team member’s duties and caseloads, adjusting as needed. Applies appropriate staffing models to delivery of patient care.
  • Assists CNA scheduler in reviewing CNA duties and caseloads, adjusting as needed. Applies appropriate staffing model to delivery of patient care.
  • Create an atmosphere that fosters growth, professionalism, teamwork, and efficiency of the IDT.
  • Seeks out challenges and change, understanding change management.
  • In coordination with discipline specific leadership, prepares performance appraisals including ride along visits, educational requirements, and goals for direct reports.
