POSP - CNA-In ADHC/In Home

MONDAY - FRIDAY = 8:30 a.m. to 5:00 p.m.

The 4-hour weekend shift is required monthly, and you can sign up to choose dates and times.

Job Title: CNA-In ADHC/In Home

FLSA Status: Non-Exempt

Salary Range: TBD

Job Summary: Performs patient care and activities in the Adult Day Health Center and/or in the home as directed. When in the center, reports to the Clinical Services Director/Adult Day Health Coordinator. When providing in-home services, reports to the PACE of the Southern Piedmont Home Care Agency Supervisor.

Specifications

Education: High school diploma or equivalent.

Experience: 1-2 years working as a CNA, with at least one year of experience working with the frail elderly population. Able to prepare simple meals and perform personal care and housekeeping tasks.

Licensure, Registry, or Certification Required: Currently listed as CNA (I or II) with NC Nurse Aide Registry and are subject to occupational licensing laws. They must have no substantiated findings of neglect, abuse, or misappropriation of resident’s property in a nursing home. A valid driving license with personal vehicle insurance is required. A dependable means of transportation is mandatory. Must meet competency testing requirements consistent with the appropriate occupational licensure laws and regulations.

Special Training: Good driving record; able to communicate verbally and in written form; able to lift and turn participants; knowledge of and care for adults with chronic health conditions. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by PACE of Southern Piedmont and approved by CMS before working independently.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact

Key Responsibilities: (*denotes an age-related skill or task)

· ADHC/Home assists with tasks on a care plan for Participants in the center and/or home that the appropriate professional personnel has taught.

· ADHC/In Provide personal care to participants at PACE of Southern Piedmont or In Home: (bathing, dressing, feeding, toileting, transfers, lifting, ambulation, and incontinent care according to the participant's plan of care

· In ADHC/In Home -Serves meals according to participant’s specific diets.

· In-home-prepare simple meals according to the participant’s specific diet. Performs incidental household services, essential to the participant's care at home and according to their care plan. Service includes but is not exclusive to meal prep, housekeeping, laundry, grocery & errands, and personal care.

· In ADHC/In Assist with large and small group activities.

· ADHC/Home-Assist on outings/field trips, appointments as needed

· ADHC/In Home-Obtain vital signs and weights per care plan and as the physician orders.

· Change oxygen tanks upon the participant’s arrival at the center, document logbook changes, and monitor oxygen tank levels throughout the day.

· Home- able to change out oxygen and replace oxygen tubing as needed.

· ADHC/In Home- Responsible for reporting any changes in medical or physical status to Triage

· ADHC/Home-Performs procedures and treatments as appropriate per certification and per the participant's plan of care under the supervision of the Center nurse or Agency Supervisor

· Home- assists participant with self-administrating medications ordered by their provider and according to their care plan.

· ADHC/In Home-Attends staff meetings, complies with competencies per POSP policies

· ADHC/In Home -Documents services and observations on appropriate forms.

· On-call rotation for home care for weekdays and weekends.
