Job Description

**Job Summary:**
The Registered Nurse (RN) in the home health office is responsible for providing administrative support and coordinating patient care services under the direction of the clinical management team. This position requires strong organizational skills, attention to detail, and the ability to communicate effectively with patients, caregivers, and healthcare professionals.

**Responsibilities:**

1. **Patient Intake and Assessment:**
- Receive and review patient referrals from healthcare providers, hospitals, and other sources.
- Conduct initial patient assessments over the phone to gather necessary information for admission.
- Coordinate with field nurses to schedule initial assessments and establish care plans.

2. **Care Coordination:**
- Collaborate with clinical staff to develop individualized care plans for patients based on assessment findings and physician orders.
- Coordinate patient schedules, including visits from field nurses, therapists, and other healthcare professionals.
- Communicate regularly with patients, caregivers, and healthcare team members to ensure continuity of care.
