Registered Nurse Care Coordinator (.5 FTE)

WAGE RANGE: $33.50-$39.10/hourly


JOB SUMMARY: Catholic Charities is seeking a compassionate, trauma-informed, and culturally aware RN to serve as a part-time Registered Nurse Care Coordinator.


The Registered Nurse (RN) Care Coordinator is a part time role designed to support service provision at two of Catholic Charities permanent supportive housing programs. Both programs work with men who have alcohol use disorder and utilize a managed alcohol harm reduction model. The RNCC’s responsibilities include managing the medication assistance program offered to clients, coordinating care with external providers during incidents of hospital or TCU admission; supporting clients in meeting their health and wellbeing goals in the context of a harm reduction lens and ensure quality care is provided or coordinated and arranged. Effective communication around coordination of care with other Catholic Charities staff and community partners is a key deliverable for success in this role. Experience working with individuals living with alcohol use disorder is highly desirable.


ESSENTIAL FUNCTIONS:

  1. Medication assistance management: interfacing with providers and pharmacies and clients to ensure medications are current; ensure pre-packaged medications are reconciled with monthly the medication record; training and monitoring staff in their role in medication assistance; assisting clients in the purpose of their medications; problem solving client barriers to medication access and adherence; implementing ongoing staff training and quality improvement activities around medication assistance.
  2. In concert with the case management team, ensure that the care needed for the client is within the program’s ability to achieve and is in line with clients’ goals and best interest. This often involves educating external providers about the unique aspects of the program.
  3. Provide guidance to program staff on health issues related to severe and chronic alcohol use disorder and how those issues may impact the client’s ability to maintain their housing and well-being.
  4. Provide input to and support in the implementation of clients’ harm reduction plans.
  5. Conduct initial and ongoing health assessments to help determine clients’ physical, social, and behavioral health care needs and work with clients and staff to meet those needs.
  6. Using motivational interviewing techniques facilitate preventive care measures. Offer health education, guidance, and support to encourage self-care and assist in decision making.
  7. Work with program management to develop written protocols (e.g., for infection control, medication disposal) and a plan for monitoring adherence to the protocols.
  8. Responsible for promptly and accurately collect, enter, manage, and report client data into systems of record.
  9. Monitor staff adherence to medication assistance guidelines. Work with program management to provide ongoing guidance, re-training and support where indicated and training of new staff.
  10. Other duties as assigned

MINIMUM QUALIFICATIONS/EXPERTISE:


  • Current MN license for Registered Nurse.
  • Two to four years of direct nursing experience; experience in community-based settings
  • Experience working with underserved or vulnerable populations, including persons from diverse racial, ethnic, and economic backgrounds, as well as those with mental health and/or behavioral issues.
  • Demonstrated ability to work with diverse and/or low-income populations, including those experiencing homelessness, unstable housing, mental illness and alcohol use disorder.
  • Comfortable working in a highly autonomous role.
  • Has ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness required.
