Summary
The Community Registered Nurse
Part-time (22.80 hours/week) (Flexible with hours between the hours of 9AM-5PM Monday-Friday) Works within the Tenant Services department to provide nursing support to clients living in permanent supportive housing for the purpose of helping the tenants maintain their housing. Strives to support clients in improving their health and well-being and provides education to clients to empower them in prioritizing their own health and wellness.
Essential Functions
- Performs health evaluations, triage and care and education either in person or over the phone.
- Provides medication management to tenants who need assistance organizing and consistently taking their medication as prescribed.
- May need to assist at other programs within the Department of Tenant Services, for which will need to travel from one location to another.
- Creates relationships with community pharmacists to ensure accurate and timely delivery of medication.
- Performs first aide, wound care, & health education for tenants who come in to the program offices or are seen in their homes through home visits.
- Demonstrates timely documentation of all visits and medications administered and timely communication with doctors, team members and other appropriate parties regarding client care.
- Participates as a member of a multidisciplinary team to ensure the nursing services are provided to clients as needed, and reports health status to the team.
- Assists in discharge planning with skilled nursing facilities and hospitals by contacting medical staff and/or coaching team members in appropriate follow up with medical providers.
- Reports any unusual occurrences or concerns regarding quality of care to the Supervisor of Case Management.
- Attends trainings and meetings at staff at the Village Health Center.
- Visits homes to determine tenants’ needs, develops plan to meet needs, and provides nursing services.
· Checks the medication cabinet to ensure medications are administered as ordered, appropriately documented on the MAR, and that missed medications are documented, and required follow-up occurs
- On time completion of assigned training and policies.
- Other duties as assigned.
Qualifications
- Current CA Registered Nurse License.
- CPR and First Aid Certification.
- Must have a valid California Driver’s License and be able to qualify for insurance coverage.
