The Mission of JWCH Institute is to improve the health status and well-being of under-served segments of the population of Los Angeles Area through the direct provision or coordination of health care, health education, services, and research.

Position Purpose:

The Clinical Supervisor HIV Outreach will oversee the new DHSP Data to Care (D2C) HIV project that utilizes external databases (e.g., Enhanced HIV/AIDS Reporting System [eHARS], HIV Casewatch, etc.) to identify Person’s Living with HIV who are not in care. The purpose of this project is to build JWCH’s capacity to locate, link, engage, and retain PLWH in medical care and supportive services so they can achieve viral suppression and reduce transmission of HIV. Services include improve the linkage, engagement, re-engagement, and durable retention in HIV care, and sustained viral loads among PLWH who were: a) Diagnosed with HIV and received HIV care at the contractor’s clinic but have fallen out of care (OOC) or are at risk of falling out of care (RFC); b) Diagnosed with HIV and reside in the nearby zip codes of the contractor’s clinic who were new referrals to care (NRC), but never received care or are OOC from a clinic other than the contractor’s clinic; increase the capacity of clinics providing HIV primary care to triangulate data from internal and external databases to identify, outreach, and re-engage PLWH who are OOC, RFC, or NRC and to increase the capacity of clinics to provide intensive case management to PLWH who return to care or are RFC.

Principal Responsibilities:

  1. Collaborate with DHSP staff on D2C activities as required by the county.
  2. Provide clinical oversight for all clinic-embedded HIV Engagement Specialist staff as they develop client engagements and care plans.
  3. Lead routine case conferences with relevant staff and support the implementation of the D2C program.
  4. The Clinical Supervisor will ensure that staff focus on priority populations that include Black and Latinx MSM living with HIV; Persons under age 30 living with HIV; Transgender persons living with HIV; Persons living with HIV who inject drugs and/or use methamphetamine, and Cisgender Black and Latinx women living with HIV.
  5. Provide Clinic oversight for HIV Engagement Specialists who conducts bio-psychosocial assessments to identify medical and non-medical needs (i.e., social, emotional, or economic factors) which may interfere with regular HIV medical care utilization such as: medical co-morbidities, sexual risk behavior, substance use history, mental health status, housing, transportation, etc.
  6. Works with clinic staff to identify patients’ unmet medical needs, coordinates with clinic HIV providers, and provides referrals to address gaps or barriers to accessing and adhering to medical care.
  7. Utilizes the population health registry to manage patients and track trends in overall clinic performance.
  8. Delivers patient education, counseling, and other brief interventions to improve HIV knowledge and reduce the risk of transmitting HIV and/or acquiring other sexually transmitted infections.
  9. Lead and ensure that Engagement Specialists participate in multi-disciplinary health team conferences to formulate the overall patient care plans by interpreting for the team the psychosocial aspects of the patient’s illness.
  10. Reviews and educates HIV Engagement Specialists on performing assessments, writing care plans, and creating social case summaries.
  11. Enter and report patient-level data into ACMS Case-Watch in a timely manner.
  12. Co-develops and reviews an individual-based treatment adherence plan with each client.
  13. Maintains client charts and updates records.
  14. Maintains quality assurance standards for the program.
  15. Performs other duties as assigned.

Requirements:

  1. Master’s degree in Social Work (MSW), Master’s degree in Marriage and Family Therapy (MFT), or equivalent field.
  2. Associate of Social Work (ASW), Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) preferred but not required
  3. At least two years of experience caring for lesbian, gay, bisexual, transgender, queer, intersex, and/or asexual (LGBTQIA+) PLWH and/or educational background, that includes knowledge of STDs, HIV, Hepatitis and/or Tuberculosis within the last five (5) years.
  4. At least 1 year supervising other staff.
  5. Knowledge of substance abuse and mental health issues and treatment, and related sexual risks.
  6. Have strong interpersonal skills and ability to work well with members of the clinical and case management team.
  7. Able to work independently; think critically; excellent time-management, and ability to make sound decisions.
  8. Able to research, identify, and access community referrals.
  9. Demonstrated experience in extensive methods of follow-up and linked access to services.
  10. Understanding of client confidentiality and legal consequences of failure to abide.
  11. Have experience maintaining confidential information, understand Health Insurance Portability and Accountability Act (HIPAA) requirements, and pass HIPAA certification training for HIPAA compliance.

Preferred:

  1. Experience with ACMS Case-Watch and Electronic Medical Records preferable Next-Gen.
  2. Experience in working with patients who have life altering conditions.
  3. Organized, able to maintain client files, charts, and referral results.
  4. Basic computer skills, knowledge of word processing, data entry, and use of Internet and Internet research.
  5. Strong quality management skills, ensure compliance with all guidelines and regulations.
  6. Bilingual in Spanish preferred; and understanding of multicultural issues.
