HOME HEALTH COORDINATOR

Our medical practice is seeking a highly motivated and detailed oriented part time Home Health Coordinator. This individual will contribute to patient care by assisting with the following tasks:

• Work with home health companies to ensure home health orders, and plan of cares are addressed in a timely manner.

• Review visit notes and assist with ensuring documentation meets CMS requirements. Assist with assigning what home health company is the best fit to refer the patient to.

• Communicate with home health companies regarding patients in small ALF`s, ILF`s, and in-home patients.

• Monitor and maintain the Home Health portals up to date.

• Assist the Home Health Lead with any requests.

• Effectively communicate with providers regarding missing information to ensure patients receive home health services in a timely manner.

• Minimum 1 year of experience in home health or Medical field
• Excellent organizational, time management, and prioritization skills.
• Strong written and verbal communication with a compassionate and patient demeanor.
• Proficiency in Microsoft Office Suite and electronic health records (EHR) a plus.
• Ability to thrive in a fast-paced environment and manage multiple tasks simultaneously.
