Job Title: Director Patience Care Services - Hospice

Location: Orange, CA; In-Office or Hospice sites

Job Type: Full-time

Education: Bachelor's degree in Nursing required, Master's degree preferred

Experience: Minimum 5 years as a professional nurse, with 2 years in hospice/home health and 2 years in supervisory nursing

Certifications: Active RN license, Certified Hospice and Palliative Nursing Certification preferred, Basic Life Support (BLS) required

Benefits: Annual Salary of $130,000 to $150,000

Job Summary:
As the Director of Patient Care Services and Clinical Services for our Hospice Services, you will play a pivotal role in ensuring operational efficiency and delivering high-quality care to our patients. You will lead and support our clinical staff, collaborating closely with the Hospice Executive Director to achieve agency goals and maintain regulatory compliance.

Essential Functions:

  • Assist in the development and administration of agency policies and procedures.
  • Oversee implementation of patient care models and procedures to ensure high-quality care.
  • Participate in program effectiveness reviews and ensure adherence to policies.
  • Supervise, orient, and evaluate hospice staff, providing mentorship and training.
  • Ensure agency readiness for surveys and compliance with regulatory requirements.
  • Provide clinical oversight for patient care, including admissions and quality assessments.
  • Lead interdisciplinary team meetings and facilitate effective communication.
  • Monitor and approve workflow in the Electronic Medical Record system.
  • Maintain positive relationships with colleagues, patients, and the community.
  • Assist with strategic growth plans, marketing activities, and public health education events.

Education and Experience:

  • Bachelor's degree in Nursing required, Master's degree preferred.
  • Minimum 5 years of nursing experience, with 2 years in supervisory roles and 2 years of hospital or hospice/home health experience.
  • Proven track record in clinical management and staff education.

Physical Demands and Working Conditions:

  • Requires standing, walking, sitting, and manual dexterity.
  • Ability to lift up to 50 pounds and occasional travel.
  • May encounter communicable diseases and various environmental hazards.

Management/Decision Making:

  • Apply guidelines and sound judgment to make departmental decisions.
  • Solve moderately complex problems based on precedent and reasonableness.

Knowledge and Skills:

  • Extensive knowledge of clinical operations and regulatory standards.
  • Strong time management, communication, and human relation skills.
