The Participant Engagement Supervisor is to direct, coordinate, and manage the daily activities of the Health Promoters and/or Healthcare Navigators on the Participant Engagement team in Boston. The Participant Engagement Supervisor will perform Health Promoter duties alongside other Health Promoters, contracted UM/CM partners, and community healthcare practitioners to facilitate communication and coordinate care to improve health outcomes. The focus of the team is to educate and engage all participants as well as those targeted for self-management and individualized care coordination. The Supervisors manages the Health Promoter team that works with participants through face-to-face interactions and telephonic follow up and works with participant leaders to expand participation in and knowledge of Fund programs and benefits.

ESSENTIAL JOB FUNCTIONS AND DUTIES

  • Trains, develops, and supervises Health Promoters to effectively recruit participants into and maximize their participation in Fund self-management programs and individualized care coordination designed to improve health outcomes. Identifies areas of strength, areas for improvement, and professional development plan and monitoring program.
  • Monitors team goals and audits team productivity and performance to ensure team is meeting metrics.
  • Oversees and reports on work and outcomes in CarMa (salesforce platform developed by UHH to collect participant engagement data).
  • Manages team workflows by delegating and assigning work to staff.
  • Participates in planning for and execution of national and regional trainings, and assists with developing and coordinating programs, policies, and procedures.
  • Coordinates with contracted UM/CM partners and healthcare practitioners in the community to optimize health outcomes and participant experience while protecting the Fund’s resources (achieving the “triple aim”).
  • Plays a key role in planning and running new or existing member events with leadership collaboration (e.g., enrollment, benefit changes, site visits).
  • Evaluates effectiveness of programs and adjusts policies, procedures, and processes for continuous improvement. Plans calendar and priorities for participant engagement work in collaboration with leadership.
  • Plans, analyzes, and evaluates programs and services, operational needs, and fiscal constraints.
  • Demonstrates the Fund’s Diversity and Inclusion (D&I) principles in their conduct with employees; partners with Participant Engagement leadership and staff to create a safe and inclusive culture with equitable opportunities for success and career growth.

ESSENTIAL QUALIFICATIONS

  • 5 - 7 years of direct experience minimum (as a Fund Health Promoter or as a Healthcare Patient Advocate/Care Coordinator in similar organization).
  • 1 - 2 years of supervisor experience.
  • Working knowledge and experience as lead-level healthcare patient advocate/care coordinator or service sector labor union/ healthcare community organizer.
  • The ability to travel 15% - 25% travel (up to 50% travel depending on region).
  • Preferred fluency (speak and write) in Haitian Creole, Spanish, or other language common to our participants in the region.

Salary range for this position: $89,100 - $111,400. Actual base salary may vary based upon, but not limited to: relevant experience, qualifications, expertise, certifications, licenses, education or equivalent work experience, time in role, peer and market data, prior performance, business sector, and geographic location.

Work Schedule (may vary to meet business needs): Monday to Friday, 7.5 hours per day (37.5 hours per week) with Hybrid Work Schedule (3 days in Office / 2 Days Remote)
